When you come to see us for the first time, we’ll treat you like you’ve been with us for years. You’ll be greeted with a smile, we’ll help organize your paperwork, and then we’ll give you a short tour of the office. After that, you’ll be taken to the back where you’ll get to meet your dentist. After a short chat, they’ll take some x-rays and perform a cleaning and comprehensive oral exam. From there, they’ll formulate your personalized treatment plan to meet all of your long term dental needs.
Before your first appointment, we ask that you fill out a few forms so we can learn some basic information about you. We know that filling out paperwork in an office while wanting your appointment to begin as quickly as possible can be frustrating, so we’ve included our forms here as a convenient download. Simply click on the links, print the forms, fill them out, and bring them to your appointment to help things get started!
Dental insurance has played a role in helping patients obtain better care. As a courtesy, we will fill out and file insurance forms for you. We will do all we can to ensure you receive maximum benefits, but bear in mind that your insurance is a contract between you (or your employer) and a dental insurance company. Benefits received are based on the terms of the contract between you (or your employer) and the dental insurance company – NOT your dental office. We strongly urge you to read the policy so that you are fully aware of coverage and any limitations of the benefits provided. In any case, we believe that insurance coverage should not stand in the way of your achieving optimal oral health. Please reach out to our office to verify if we are in-network with your insurance.
